Use advanced questions, logic and preview in the form creator

The form creator is the editor you use to build questions for visitor check-in fields, custom pages, feedback forms, check-out pages and Digital Register forms. This article shows you how to group questions, collect repeated answers, show or hide questions with rules, and test the form before you save.

Before you start

  • You need permission to update visitor types (for check-in fields and custom pages), to update settings (for feedback and check-out forms), or to create and update register forms (for Digital Registers).
  • Menus and controls depend on which products are turned on for your company and on your access.
  • Availability: if this option is greyed out or missing, contact your Vizmo account manager.

Open the form you want to edit

Form Where to open it
Check-in Fields Visitors > Settings > Visitor Types, select the visitor type, then open Check-in Fields.
Custom page The visitor type's Custom Pages tab, then Add Page or the pencil icon beside an existing page.
Feedback form or check-out page Visitors > Settings > Check-out, turn on Visitor Feedback or Check-out Page, then select Edit Feedback Form or Edit Check-out Page.
Digital Register form Digital Registers > Settings, select View Forms beside the register, then Add Form or the form's pencil icon.

Choose the right question type

Add questions from the toolbox on the left. Pick the simplest type that fits.

Question type Use it when
Single-line text, long text The answer is free text, such as a name or a comment.
Radio group, dropdown, checkbox, tag box The visitor picks from a list. Radio group and dropdown allow one choice; checkbox and tag box allow several.
Boolean, rating, ranking You need a yes or no answer, a score, or choices put in order.
Multiple text One question needs several short text boxes, such as a name and a phone number.
Date, time, month, week, date-time The visitor enters a date or time.
Single-choice matrix The visitor picks one answer per row of a grid, such as rating several services.
Matrix dropdown, dynamic matrix The visitor fills in a grid. In a dynamic matrix the visitor adds rows as needed.
Panel You want to group related questions together.
Dynamic panel The visitor must enter several sets of the same details, such as several items of equipment, each with a name and serial number.
File upload The visitor attaches a file.
Calculated expression You want a value worked out from other answers.

HTML, image, image picker, signature pad and slider elements are not available. Each editor holds one page. If you need more check-in screens, add separate custom pages in the visitor type settings.

Group or repeat questions

  1. Add a Panel to group related questions, or a Dynamic Panel when the visitor needs to enter several sets of the same details.
  2. Add questions inside the panel. A dynamic panel must contain at least one question before you save.
  3. Select each answer question and set its title, choices and Required setting. A panel is only a container, so set Required and the visitor field options on the questions inside it.
  4. For a matrix dropdown or dynamic matrix, set up the cells that need answers. A partly completed required row can still fail validation.

Note: On visitor check-in fields and custom pages, the email and badge options apply to a matrix answer as a whole, not to individual cells. Show on Badge is available only for Check-in Fields. Employee Selection and Multiple Employee Selection are available for custom pages and Digital Register forms; they use employee search instead of a list of choices you type.

Show, hide or require questions with Logic

The Logic tab is available for custom pages, feedback forms, check-out pages and Digital Register forms. It is not available for Check-in Fields.

  1. Create the questions the rule will use. For example, add a choice question called Bringing equipment? and a text question called Equipment details on the same form.
  2. Open Logic and select Add New Rule.
  3. In the condition section, choose the question, the comparison and the answer that activate the rule, such as Bringing equipment? equals Yes.
  4. In the actions section, choose Show/hide question and select Equipment details. The question appears when the condition is true.
  5. To also require that answer under the same condition, add a Make question required action for Equipment details.
  6. Select Done to finish the rule.

Enable/disable question is another action: the question can be edited only when its condition is true. Keep conditions tied to questions on the form you are editing. Finish or remove incomplete rules before you leave the Logic tab.

Check and preview the form

  1. Give the page a title and every question a title. A page title is required for custom pages, feedback forms, check-out pages and Digital Register forms.
  2. Turn Required on for questions that must be answered. For questions inside panels, multiple-text questions and matrices, check the nested answers too.
  3. Open Preview. Try leaving required answers empty, completing only part of a matrix row, adding repeated entries, and trying each branch of your rules.
  4. Complete the preview, then select Preview Again to test another case.
  5. Save from the page or panel you opened the creator from. This is Save for a visitor type, custom page or feedback form, Save Page for a check-out page, and Create Form or Save Changes for a register form.

Form creator Preview tab with the Preview Again button

Preview does not save the form or create a visitor check-in or register entry. The Check-in Fields preview shows all configured questions on one page, so it does not show which fields are hidden in the visitor flow. The creator has no separate validation section: use the Required settings and the Logic actions described above.

Troubleshooting save errors

  • Page title is required. Enter a title for the page.
  • Add titles to all questions before saving. Give every question a title, including questions inside panels.
  • Add at least one question before saving. The form cannot be empty. Add an answer question.
  • Dynamic panels must contain at least one question. Add a question inside the empty repeated section.

Limits to check before saving

  • For visitor fields, Required, Hide for Visitors and Read-only for Visitors cannot be turned on together for the same question.
  • Show in Email is limited to three non-default fields across Check-in Fields and custom pages combined. Show on Badge is limited to two non-default Check-in Fields.
  • Feedback, check-out and Digital Register forms do not show the visitor-specific email, badge, hide, prefill or read-only options.
  • File questions accept JPEG, PNG, WebP, PDF, CSV and XLSX files. A question can narrow this list but cannot add other file types.
  • Built-in check-in fields such as first name and last name cannot be renamed, removed, moved or converted. Custom questions under Additional Details can change type where that option is shown.
