Set up a health declaration

A health declaration is a short Yes/No questionnaire that visitors answer as part of check-in. You choose whether it is shown, what the title is on the kiosk, and which questions are required.

Before you start

  • You need to be an Admin or Location Admin to change these settings. Other roles can only view them.
  • Availability: if this option is greyed out or missing, contact your Vizmo account manager.
  • Write each question so that it can be answered Yes or No.

Set up the health declaration

  1. Sign in to the Vizmo portal and select your location.
  2. Go to Visitors > Settings > Health Declaration.
  3. Under Health Declaration Settings, turn on Health Declaration.
  4. In Title of the health declaration screen shown on the kiosk, enter the heading that visitors see on the kiosk.
  5. Under Health Declaration Form Fields, enter each question in Title. Turn Required on if visitors must answer it. New questions are required by default. Keep the list short, about 10 questions or fewer, so check-in stays quick.
  6. Select Add Field to add another question. To remove one, select its Delete (bin) icon.
  7. Select Save at the top right of the page. The portal shows "Settings saved successfully."

The page starts with default questions that you can change. If you delete every question and save, Vizmo puts the default questions back.

What visitors see

  • On the kiosk: a screen with your title and the numbered questions. Visitors answer Yes or No, and must answer every required question to continue. A returning visitor who has already completed a declaration is not asked again.
  • On their phone (Touchless): a screen headed "Health Declaration" with a Yes/No choice for each question and a statement that their answers are correct to the best of their knowledge. The title you set above is not used on the phone.
  • Invited visitors: about 24 hours before the visit, Vizmo sends a message with a link so they can complete the declaration in advance.
  • When staff check in a visitor: the same questions appear under Health Declaration in the check-in form in the portal.

See a visitor's answers

  1. Go to Visitors > Visitor Log and open the visitor's row.
  2. On the Details tab, find the Health Declaration section.

Things to know

  • A visitor type can override the location setting. Open it from Visitors > Settings > Visitor Types, go to its Override Settings tab and select Click to override in the Health Declaration card. A visitor type can switch the declaration on or off and use its own title. The questions are the same for the whole location.
  • After you change the questions, do a test check-in on your kiosk to confirm the flow.
  • There is no separate health declaration approver. The questionnaire records the visitor's answers and does not start a health approval. To have someone approve visitors, set up an approval. See Set up approvals for visitors and invites.

Troubleshooting

  • The questionnaire does not appear. Check that Health Declaration is on and saved, and that the visitor type is not overriding it with the setting off. Then start a new check-in to test.
  • A returning visitor was not asked on the kiosk. This is expected if they have already completed a declaration.
  • A message asks for the required details when you save. Enter text in the title and in every question.
  • You cannot change the settings. Only Admins and Location Admins can edit them.
